Adult Program Support Specialist - Part Time

Intro:
The Adult Day Program Activity Assistant is primarily involved in the daily interactions with participants enrolled in the Adult Day Program. They will be expected to assist their coworkers in a variety of daily activities designed to enhance and maintain cognitive, physical, social and emotional well-being of each participant. 

JOB DUTIES: 

  • Ensure that participants are engaged in activities of their choice while in the center.  Offer assistance at every opportunity, without being asked, and with an attitude that will encourage and inspire each participant you work with.
  • Be familiar with each enrollee’s Individualized Care or Service Plan and assist with implementation of their requested choices each day. Demonstrate an understanding of the key components of Person-Centered Care - Respect, Dignity, Confidentiality, Responsibility, Coordination, Experience & Goals and how they apply to participants enrolled in Day Services.   
  • Provide support to the Activity Specialist in preparing, carrying out and cleaning up after planned daily activities.
  • Knowledge, understanding and willingness to learn the characteristics of the variety of diagnosis’ represented within the center. Keep informed of all legal requirements governing adult day services and participate in all required training sessions.
  • Assist individuals at mealtime and with snacks and other requested food and drink.  You should assist consumers in feeding according to their personal preferences. Be willing to adhere to CACFP guidelines when serving individuals.
  • Assist participants with toileting, including transferring and changing of undergarments or clothes if needed.
  • Maintain all necessary records and reports on individuals enrolled in the program including but not limited to daily, and monthly notes, sign in sheets, incident reports, etc.
  • Develop and maintain supportive relationships with caregivers. Provide support, advocacy, and resources to caregivers upon request and determine eligibility when necessary.
  • Act as a team player with coworkers to provide leadership, assistance, and coverage for one another when working with individuals throughout each day.
  • Attend all in-service training and meetings as requested by the Director.
  • Other duties as assigned by the Director of Adult Services


 


Qualifications:
  • High School Diploma (Associates Degree in Human Services or related preferred)
  • At least one-year experience working with individuals with disabilities.
  • Commit to the principals and values of Person-Centered Care philosophy utilized in Adult Day Services
  • Must be able to take initiative while encouraging participants to engage in daily activities
  • Demonstrate creativity & patience
  • Willingness to be flexible and open to feedback when working with participants
  • Can juggle multiple tasks throughout the day
  • Work within a team environment
  • Willingness to adhere to Signal Centers policies & procedures
  • The ability to read, write, and use a computer
  • Be able to communicate effectively with participants and coworkers.

PHYSICAL REQUIREMENTS: 

The physical requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

Must be able to stand for long periods of time; must be able to bend over; lift at least 50 lbs; push a manual wheelchair; must be able to use both hands; must have fine muscle control.  Must be able and willing to assist clients with transfers, adjustments and possible hygiene issues when necessary.  Physical examination including TB skin test or chest x-ray is required prior to employment.
